Both a recent summary of alcohol's relationship to cancer and updated guidance for practitioners treating high blood pressure (hypertension) recognize the benefits of reducing alcohol consumption, including all the way to zero. But neither uses the heightened rhetoric that there is "no safe level" of alcohol consumption, embraced by certain advocates. And though heart health experts did change their guidance by endorsing abstinence for certain patients, the document's actual language is far less absolutist than some early press coverage of the change suggests. Cancer Links Less Clear at Low Levels, JAMA Summary Confirms Published last week, a summary of evidence about alcohol and cancer has been trending on JAMA's website ever since. The topic is hot. But the 2-page paper breaks no new ground. After reviewing the types of cancer that have been linked to alcohol consumption and the possible mechanisms for such links, it succinctly confirms that "cancer risk increases with higher amounts of alcohol consumption." The Yale-based authors found "challenges" for clearly defining risk based on existing research, "particularly at lower consumption levels." Ultimately, "heavy alcohol consumption is associated with the greatest risk," while conclusions "differ" for light and moderate drinking. Ref 1Researchers point to the findings of the late-2024 report from a committee of scientists appointed by the National Academies (NASEM), originally designed to inform the next edition of the Dietary Guidelines for Americans. That review found associations between low levels of consumption and breast cancer among women, but inconclusive results for most other cancers, recall. (By the way, this brief new paper does not acknowledge the findings of a dueling panel gathered by the Interagency Coordinating Committee of the Prevention of Underage Drinking.) Earlier reviews offered conflicting findings depending on methodology, though. The paper recommends clinicians point patients to the existing Dietary Guidelines before concluding that "any daily alcohol consumption is associated with certain cancers, and the risk of cancer increases with higher alcohol intake."AMA Recommends "Reducing or Eliminating Alcohol Intake" for Hypertension This week, a wide array of organizations associated with heart health, led by the American Heart Assn, updated guidance for clinicians seeking to help patients prevent and treat high blood pressure. Alcohol plays a relatively minor role in the highly detailed, 100+-page document. However, as in past editions, it acknowledges that "alcohol overuse" is a leading cause of high blood pressure alongside high-sodium and low-potassium diets. So "reducing or eliminating alcohol intake" is one of numerous "lifestyle changes" that "are strongly recommended to prevent or treat elevated blood pressure and hypertension." Ref 2Abstinence is safer than even the light consumption of 1 drink per day, the guidelines point out. Lowering consumption from 1 to 0 drinks per day can lower risk of hypertension by 11%, while increasing consumption to 2 drinks per day can increase risk by the same amount, the document explains. However, it also acknowledges that both the risks for high blood pressure and the benefits of reducing consumption are more apparent in individuals with heavier baseline consumption. Still, since "there are no harms identified with alcohol reduction," authors conclude that "aiming for abstinence appears to be optimal."
